Abstract
To elucidate clinical and pathological characteris- tics, 2,001 adult patients with various types of glo- merulonephritis(GN) from a single center in Korea were studied retrospectively, and results were com- paired with those previously reported. In general, the mean age at the time of biopsy was 32.7(range 15-81), and male to female ratio was 1.3:l. Amomg these, 1,416(70.8%) patients were be- long to primary GN and 585(29.2%) patients were belong to secondary GN. In primary GN, the most common pathologic dia- gnosis was minimal change lesion(MC, 26.3%), which was followed by IgA nephropathy(IgA N, 25.1%), focal glomerular sclerosis(FGS, 13.9%), membranous nephropathy(MN, 13.1%). Percent incidence of MC and FGS were out-numbered to MN until the age of 49, if these patients were grouped by decades. The mean age of MC was lower(29.3yr), compaired to IgA N(31.4yr), FGS(31.4yr), and MN(41.5yr). The incidence of nephrotic syndrome in MC(82.0%), FGS (64.4%), MN(82.8%) were higher than that of IgA N(18.5%). Major causes of secondary GN were lupus ne- phritis(LN, 47,2%), Hepatitis B-virus associated GN (HBGN, 31.6%), Henoch-Schonlein nephritis(8.2%), acute poststreptococcal GN(4.8%), amylodosis(3.1%) and others(5.1%) in descending order. Male predomi- nance was noted in HBGN(3.9:1) and amyloidosis (2.6:1), while in LN, female were much more com- mon than male(1:10). Various factors including ethnic difference has been known to affect prevalence of GN. These pre- sent data showed that adult GN patient in Korea was different from western patients especially in following points. Firstly, MC was more than IgA N or MN. Secondly, HBV infection was the most com- mon secondary GN in male.
