| Abstract |
|
We report a case of hyperchloremic metabolic acidosis with normal anion gap accompanied by hyper- kalemia and hyperreninemic hypoaldosteronism in a 56-year-old man who has been suffered from diabetic nephropathy and was receiving Captopril. He was diagnosed as Type 4 renal tubular acidosis due to Captopril administration. When Captopril was with- drawn, elevated serum potassium level, metabolic acidosis and hyperreninemic hypoaldosteronism were improved. This case demonstrates that Captopril can cause hyperchloremic metabolic acidosis with normal anion gap accompanied by hyperreninemic hypoaldoster- onism & hyperkalemia. |
