신장이식 110예에 대한 임상적 고찰

Abstract
Between August 1978 and December 1988, 110 renal transplants were performed at Kyung Hee University Medical Center. On January 1 1989, 869 of the recipi- ents were alive and 72% had functioning graft. The results of the transplantation are analyzed according to various factors known to influence the outcome of trans- plantation. 1) The most common underlying disease of ESRD was chronic glomerulonephritis, hypertension, chronic pyelonephritis and so in order of frequency. 2) The mean age of recipients and donors were 35.5 years and 44.4 years. The male to female ratio was 2.6: 1 and 1:1.25 respectively. 3) Among 110 patients, 94 patients received kidney from living related donors (LRD), 15 from living non- related donors (LNRD) and 1 from cadaver donor. 4) The overrall patient survival rate was 88% at 12 mon, 79% at 24 mon, and 71% at 36 mon. The corre- sponding allograft survival rate was 70%, 71 % and 62 % respectively. 5) In the Aza group, the allograft survival rate at 12 mon, 24 mon, 36 mon was 74%, 70% 62% and in the CyA group 91%, 80%, and 80%. In LRD, there was no differ- ence in the graft survival rate between Aza and CyA group but in LRD allograft survival was higher in CyA group than that in Aza group. 6) The graft survival rate was higher in HLA identi- cal group than in haploidentical or mismatched group. In HLA identical LRD recipients, there was no differ- ence in the graft survival rate between two groups, however in HLA haploidentical LRD recipients, graft survival rate was higher in CyA group than that in Aza group. 7) Donor age had no effect on allograft survival and did not affect the rate of increase in serum creatinine between 1 year and last follow up. 8) After conversion from CyA to Aza, rejections usually occurred within 3 months and had been treated successfully with methylprednisolone. There was no significant difference of 1-year graft survival rate between CyA group and CyA to Aza conversion group.
