만성요독증 환자에서 혈액투석중 안압의 변화

Abstract
To evaluate the changes in intraocular pressure (IOP) during hemodialysis and to determine whether a correla- tion exists between changes in plasma osmolality and IOP, we measured IOP with a Goldmann applanation tonometer in 18 uremic subjects at hemodialysis off day and in 35 dialyzed uremic subjects before and within 30 minutes after hemodialysis. 1) The mean IOP of 53 uremic subjects was not signif- icantly different than that of normla subjects (14.4±2.1 vs. 13.73.6mmHg; P=N.S.) 2) The IOP of 35 pre-dialytic subjects was 14.1±2.4 mmHg in the right ere and 14.5±2.6 in the left eye. The IOP of the post-dialytic subjects was 16.1±3.2 mmHg in the right eye and 15.6±2.7 mmHg in the left eye. The mean IOP of both eyes in the post-dialytic subjects increased significantly than that of pre-dialytic subjects (15.8±2.8 vs. 14.3±2.4 mmHg; P=0.02) 3) The increase in mean IOP 3 mmHg or more than the value of pre-dialysis was observed in 11 subjects (31. 4%) 4) There was a significant inverse correlation between the changes of mean IOP and plasma os- molality during hemodialysis (r= 0.415, P=0.02). Based on the above finding, it is suggested that the hemodialysis may increase IOP possibly through the disequilbrium of osmolality between the plasma and intraocular chamber in twice weekly dialyzed uremic patients.
