성인 신증후군에서 원발성 소성사구체경화즈의 임성상

Abstract
Study Objective: To know the frequency, the clinical and pathological characteristics and the response to treatment of focal glomerulosclerosis with nephrotic syndrome in Korean adults who were admitted to the Department of Internal Medicine of Seoul National University Hospital. Design: Case-control study+Descriptive study Setting: Tertiary care center Patients: 50 adults with primary focal glomeruloscler- osis with nephrotic syndrome. Only 30 patients could be followed over 25 weeks. Interventions: Initially prednisolone 1 mg/kg (body weight) was instituted once a day for 28 days. According to the response, dosage was adjusted, and in case of no response, cyclophosphamide and dipyridamole were added. Main Results: The patients with focal glomeruloscler- osis constituted 17% of the patients with nephrotic syndrome due to primary glomerular disease. Focal glomerulosclerosis with nephrotic syndrome was most prevalent in 15-24 aged adults and showed male prepon- derance. Crescent was seen in 8%, but it didnt indicate poor prognosis. Immunofluorescent microscopy dis- closed deposits of fibrinogen, IgM, C, and IgG, but not IgA. Out of 30 patients who were followed for over 25 weeks, complete remission was observed in 7 patients, partial remission in 8 and no response in 15. Relapse was encountered in 9 patients and 3 patients progressed. Degree of glomerulosclerosis was statistically different between remission group and no response group. Conclusion: The more severe the degree of glomer- ulosclerosis ws, the worse the prognosis would be.
