Polymethylmethacrylate (PMMA) 투석막이 혈장보체 활성화에 미치는 영향

Abstract
Complement activation represents one aspect of blood-materials interactions and may be considered as an index of bioco@mpatibility because products of complement activation have several biological actions, such as leukoaggregation, histamine release from mast cells, and increase in pulmonary capillary permeability. The ability of polymethylmethacrylate (PMMA) rnembrane to activate complement causing cause neutropenia and hypoxia was studied pro- spectively in ten chronic hernodialysis patients using PMMA membarane. We performed serial measure- ments of arterial blood gas analysis, leukocyte counts and plasma complement during hemodialysis. 1) Plasma levels of C3adesArg & C5adesArg were nonsignificantly elevated at 15 rninutes of hemodialysis and returned to normal within 4hours. 2) The leukocyte counts were slightly decreased at 15 and 30 rninutes of hemodialysis and then gradually reutrned to basal levels. 3) Pa02 was constant during hemodialysis with PMMA membrane in both acetate and bicarbonate dialysates. 4) There was a significant correlation between leukocyte counts and plasma C3a desArg levels during hemodialysis (r=0.89, p<0.05) Thus we concluded that in the aspect of the ability to active complement activation and cause neutropenia and hypoxemia, PMMA membrane dialyzers appeared to be more biocompatible.
