| Abstract |
|
In order to determine the role of histamine (H) in the pathogenesis of uremic pruritus we measured plasma H levels by double isotope enzyme assay in 23 hemodialysis patients and 12 normal controls (NC: mean age 27.5 yrs). Of 23 patients, 13 (mean age 49.4 yrs, duration of dialysis 21.5 mos) were complaining of sever- e pruritus (PU) and 10 (mean age 41.5 yrs, duration of dialysis 32.1 mos) did not have pruritus (NPU). Plasma H concentation in PU was significantly higher than in NC (1.236±0.418 vs 0.393±0.081ng/ml, p<0.05) and was also higher than in NPU (0.550±0.273 ng/ml) alth- ough the difference did not reach statistical significance. Mast cell stabilizer, ketotifen (2 mg/d) was administer- ed to 5 patients in PU group for 7 days and plasma H decreased in 4 with symptomatic improvement. Discon- tinuation of ketotifen caused significant rebound in plasma H level with return of pruritus. Readministra- tion of ketotifen was again followed by a significant decrease in plasma level of H and symptomatic improvement of pruritus. In conclusion, plasma H level was significantly ele- vated in pruritic dialysis patients and H appears to play an important role in uremic pruritus. |
