전형적인 B형 간염에 동반된 급성 신부전증 1예

Abstract
We experienced a rare case of acute viral hepatitis B complicated by acute renal failure without precipitating causes of renal functional deterioration. A 58-year old female was admitted to out medical center due to oligur- ia. She had mild jaundice. The liver and spleen were not felt. On laboratory examination, urine gave a 2(+) test for protein and bilirubin. Serum bilirubin (T/D) was 4.3/3.1 mg/dl, and alkaline phosphatase 72 IU/L, GOT 1944 U, and GPT 2223 U. BUN was 64 mg/dl, creatinine 5.6 mg/ dl. Hepatitis B virus markers were HBsAg (+), antiHBs (- ), antiHBc (+), HBeAg (+), and antiHBe (-). The biopsy finding of the kidney was consistent with acute tubular necrosis and that of the liver was consist- ent with acute viral hepatitis. The patient received hemodialysis four times during hospitalization of 25 days. With the improvement of the hepatic function, the urine output and renal function were improved.
