Abstract |
Thrombosis is one of the important complications in nephrotic syndrome. Hypercoagulability is known as the principal contributing factor in the pathophysiologic mechanism, but the exact mechanism is under the debate. Venous thromboses are frequently encountered, but arterial thromboses are very rare complication and only about 30 or more cases have been reported in the literature. We reported the 4 cases of arterial thromboses among 360 patients with nephrotic syndrome of primary glomerulonephropathy who were admitted to Seoul National University Hospital from January 1979 to March 1986. 1) The patients were 3 men who were 49, 27 and 17 years old respectively, and one woman who was 17 years old. 2) Duration of the nephrotic syndrome to arterial thrombosis were between 5 months and 15 months. 3) One of the 4 cases had membranous nephropathy, the others minimal change disease. 4) The involved sites were aortic, femoral, renal, cerebral and popliteal arteries. 5) Both steroids and diuretics were administered in all cases. Levels of serum cholesterol were above 500 mg/dl in all cases. The levels of serum albumin were below 2. 0 g/dl except one patient who had a thrombosis of femoral artery after femoral puncture. 6) Two cases were treated by anticoagulant therapy only, and the others were treated by both thrombectomy and anticoagulant therapy, and all of them were im- proved without further events. |
