스테로이드에 반응하는 IgA 신병증에 동반된 신증후군

Abstract
We presented three cases of IgA nephropathy which were accompanied by steroid responsive nephrotic symdrome. Generally, severe proteinuria in 1gA nephropathy is regarded as poor prognostic signs indicating progressive renal insufficiency with severe renal histopathologic changes such as advanced glomerular sclerosis and crescent formation. But our cases revealed minor his- topathologic changes inspite of massive proteinuria. Furthermore, these cases took a clinical course mimick- ing that of minimal change nephrotic syndrome, char- acterized by steroid responsive selective proteinuria, frequent relapses (especially in case 3.), absence of hematuria, and absence of azotemia during the period of follow-up available. However we could diagnose these cases as IgA nephropathy by confirming the predomi- nant diffuse mesangial IgA deposits immunopath- ologically. In conclusion, we experienced three cases of atypical IgA nephropathy with steroid responsive nephrotic syndrome which had minor histopathologic changes. This implies that the use of corticosteroid may be jus- tified where the nephrotic syndrome is due to IgA nephropathy with minor histopathologic change.
