만성 혈액투석 환자에서 혈액투석막이 혈청 Interleukin - 2 수용체와 B2 - micrglobulin 농도에 미치는 영향

Abstract
Background
The soluble interleukin-2 receptor (Sil- 2R) is a marker of the activation of an immune system and is used as the characteristic marker of an activation of T lymphocyte. The beta-2 microglobulin (m) is a low molecular protein causing an amyloidosis in chronic hemodialysis patients. In order to compare the biocompatibility of both cuprophan and PMMA membrane, the effects of different dialysis membranes (cuprophan vs PMMA) on serum slL-2R and Bm levels were evaluated. Methods: The fifteen patients receiving chronic hemodialysis for 17 months were selected. The study were performed in a prospective crossover design study. All patients were dialyzed with cuprophan membrane for 5 weeks (phase I). After 5 weeks, the patients were switched to 5 weeks of PMMA membrane (phase II), then were returned to 5 weeks of cuprophan membrane (phase III). Blood samples for Sil-2R and Bm were drawn at the beginning and end of each phase and control samples were obtained from 10 healthy adults. Results: 1) Serum Sil-2R and pgn levels in patients group were significantly higher than those in control group (Sil-2R: 495±105 vs 1564±712 U/ml, Bm: 1.17±0.18 vs 28.32±11.18 pg/dl, p<0.05). 2) Serum Sil-2R and Bm levels at the end were significantly higher than those at beginning in phase I (cupro- phan membrane) (Sil-2R: 1564±713 vs 1928±542 U/ml, gm: 28.32±11.18 vs 31.89±8.38 mg/dl, p<0.05). 3) Serum Sil-2R and B levels did not change significantly in phase II (PMMA membrane) Sil-2R: 1902± 620 vs 1985+636 U/ml, B,m: 33.76±10.46 vs 32.34±10.70 pg/ dl, p>0.05). 4) Serum Sil-2R and Bm levels at the end of phase III (cuprophan membrane) were significantly higher than those at beginning (Sil-2R: 1744±508 vs 1958±634 U/ml, Bm: 35.00±11.29 vs 38.27±12.42 mg/dl, p<0.05). Conclusion: PMMA membrane elevates serum Sil-2R and Bm levels less than cuprophan membrane, then it seems that PMMA membrane has more biocompatibility than cuprophan membrane. But further study was requested.
