만성 신부전 환자에서의 의 Insulin - like Griwth Factor Binding Proteins 특성

Abstract
Insulin-like growth factor, IGF-I and IGF-II, are pro- teins that promote cellular growth and differentiation of various organs including the kidney. Each peptide inter- acts with high-affinity receptors and also has high affinity for a family of insulin-like growth factor bind- ing proteins (IGFBPs). Altered serum and urine IGFBP patterns in children with chronic renal failure (CRF) have been described. In this study, we have evaluated serum and urine IGFBP profilles in chronic renal failure patients (n=10), by using western ligand blot(WLB). Most patients with chronic renal failure showed decreased intact serum IGFBP-3, increased serum IGFBP- 2, markedly increased urinary IGFBP-1 and totally absent urinary IGFBP-3. To evaluate altered IGFBP profiles, we performed IGFBP-3 protease assays with sera and urines from renal failure patients and normal controls. Control serum and sera from chronic renal faliure did not showed protease activity (defined by the ability to degrade ['"I]-IGFBP-3). The significant protease activity was found in urines from patients. Although control urine had only minor protease activity, the proteolytic pattern of IGFBP-3 was very similar to that found in urine from renal failure patients. Although the exact role of these protease was not established, this protease may play a role in the regulation of growth by modulating IGF availability in a tissue specific manner.
