횡문근융해증에 동반된 급성 신부전의 임상적 고찰

Abstract
Five patients with acute renal failure accompanying rhabdomyolysis were studied and following results were obtained. 1) The patient consisted of 4 males and 1 female with mean age of 32 2±11 9 years. 2) Rhabonyolysis was 19% cause of acute renal fail- ure at Ewha Womans University Hospital and trauma was the main etiologic factor of rhaMomyolysis. 3) Serum creatinine phosphatase, serum myoglobin, orthotoluidine test of urine were useful diagnostic method in rhabdomyolysis. 4) Initial increase in hematocrit indicated volume contraction and during the clinical course, blood chemistry showed severe hyperkalemia, hyperphosphatemia, hyperuricemia and hypocalcemia. 5) All five cases were oliguric acute renal failure. Duration of oliguric phase was 7 8±2 3 days and average 2. 6 times of hemodialysis were required during the clinical course. 1 case expired during the clinical course. With above results, in patients who develops acute renal failure, especially in crushing injury, rhab- domyolysis should be considered as an etiologic factor.
