소아 신증후군환아에서 cyclosporin A 의 신독성에 대한 고찰

Abstract
Cyclosporin A is a new therapeutic ageot in pediatric nephrotic syndrome which dose not suppress cellular mitosis and not have cytotoxicity to lymphocytes. Therefore, this study was conducted to find out therapeutic effects and side effects of Cyclosporin A in steroid dependent and steroid resistant nephrotic syndrome suffered from side effects of conventional im- munosuppressants such as corticosteroid and cyclophos- phamide and Henoch-Schonlein prupura nephritis and IgA nephritis associated with nephrotic syndrome. The results obtained from this study are summariaed as follows. Cyclosporin A was effective as an alternative thera- peutic agent for MCNS and other types of nephrotic syndrome, when steroid and other immunosuppressants were ineffective or showed toxicity. In both steroid sensitive and steroid resistant groups of MCNS, the incidence of relapse decreased significant- ly. In the 16 cases of MCNS, 6 cases of MGN and 6 cases of HSPN showed significant improvement in serum totoal protein, albumin and 24hr urine protein excretion. In the 3 cases of FSGS and cases of IgAN, showed significant improvement in seurm albumin level. Sequential renal biopsy showed toxicity of CsA therapy such as tubular atrophy and interstitial fibrosis in 2 Of 34 (6%). Other side effects of CsA therapy included hirsutism (47%), hypertension (15%), chickenpox (15%), elevated SGOT/SGPT (12%), paresthesia (3%) and tremor (3%).
