말기 신부전증 환자에서 B형간염 백신에 대한 반응군과 비반응군 사이의 면역상태 , 조직항원 및 이식신 생존율의 비교 연구

Abstract
Clinical comparative studies were carried out in chronic renal failure patients who were responding (Group I: 15 patients) and nonresponding (Group II: 15 patients) to hepatitis B vaccine, and the following results were obtained.: 1) There were no significant differences in age, sex, duration of dialysis and amount of transfusion between the two groups. The underlying diseases were CGN in all patients of both groups. 2) The distribution of T-lymphocyte subsets and HLA-DR positive cells in peripheral blood were not different between the two groups, 3) Although statistically not significant, the Con-A induced suppressor cell activity was slightly higher in group I (28.2±9.7%) than in group II (23.0+8.2%). Like- wise, the PHA-induced T cell stimulation was tended to be higher in group I (85.2± 100.0) as compared with than group II (47.0±63.0). 4) Serum IgG, IgA, IgM, C3 and C4 levels were not significantly different between the two groups. 5) HLA antigen frequencies in group II were compar- ed with those of 200 living unrelated healthy persons with respect to 24 antigens of HLA-A locus, 51 antigens of HLA-B locus, 11 antigens of HLA-C locus and 20 antigens of HLA-DR locus: In group II, the relative risks of HLA-A26, A30, A34 and A24 antigens were 13.58, 3.42, 2.54 and 2.48 those of HLA-B13, B40, B44 and B38 antigens were 4.41, 3.46, 3.46 and 3.02; those of HLA- Cw7, Cw3 and Cwl antigens were 1.6, 1.3 and 0.8; and those of HLA-DRw6, DR3 and DRw7 antigens were 3.55, 3.43 and 3.13 respectively. However, these values were not significantly different from those of healthy persons. 6) First and second year graft survival rates were 85. 1 and 85.1% in group I, 100.0 and 88.2% in group II and the difference was not significant statistically (p>0.05). These results suggest that no apparent difference exists in immunologic parameters and histocompatibilty antigens among chronic renal failure patients who were responding and nonresponding to hepatitis B vaccine. Certainly, more studies are required to ascertain these results.
