기종성 신우신염의 임상상

Abstract
Emphysematous pyelonephritis is rare and life threatening suppuressive infectoin of renal parenchyma and perirenal tissue. The disease which is characterized by gas formation in renal and perirenal tissue, is en- countered mainly in patients with diabetes mellitus and obstrictive uropathy. We evaluated 7 patients with emphysematous pyelonephritis admitted in Chungnam National Univer- sity Hospital (CNUH) from 1982 to 1990. The results were as follows: 1) Mean age was 50 years old and 6 of 7 patients were women. 2) All 7 patients were suffered from diabetes mellitus. And fasting blood sugar levels of all patients were above 240 mg/dl. 3) Unilateral right renal involvement of em- physematous pyelonephritis was 4 cases, unilateral left involevemt were 2 cases and bilateral involvement was 1 cases. 4) In roentgenologic staging, 5 patients were stage II and 2 patients was stage UI. 5) Pathogenic organisms were E coli in 5 patients and Klebsiella in 3 patients. In one patient, E coli was cultured from urine and Klebsiella from blood culture simultaneously. 6) 3 patients were treated witb nephrectomy and other 3 patients were managed with incision and drain- age. This 6 patients with surgical treatment were recovered completely. But 1 patients managed with medicial treatment was dead due to progressive deterioration and sepis before surgical treatment.
