신 이식 후 발생한 자가 면역 용혈성 빈혈

Abstract
Several cases of autoimmune hemolytic anemia has been reported in 1980s. Rencently, we experienced a case of autoimmune hemolytic anemia which was devel- oped on 10 days after renal transplantation during CyA, AZA, PDN administration as immunosuppressant agents. The kidney donors blood group was Rh(+) 0 and the recipients blood group was Rh (+)B. The patient showed decreased hemoglobin (3.8 gm/dl) and hemato- crit (11.2%.) level. The reticulocyte count was increased to 4.1%, indirect bilirubin level was 2.0 mg%. The poly- chromatosis, spherocytosis, and anisocytosis were obser-ved in peripheral blood morphology examination. The autoantibody that caused hemolytic anemia is confirmed as anti-B (IgG). It is supposed that this anti- body was produced by the kidney donors passenger B lymphocyte. The patient has recovered from anemia 64 days after renal transplantation and lives well with transplanted kidney.
