만성 신부전 환자에서 신이식후 심장의 기능 및 형태학적 변화

Abstract
Cardiac failure is common in patients with end stage renal disease. The effects of chronic hemodialysis on the uremic patients heart have been studied extensively. However, little is known about the effects of successful renal transplantation on the cardiac status of these patients. To determine the change of the cardiac function and morphology after successful renal transplantation, echocardiographic examination was performed in 14 patients with end stage renal disease before and after one month of renal transplantation. The results were as follows; 1) LVEDD was decreased significantly from 5.68±1.15 cm to 4.80±0.90 cm after one month of renal trans- plantation. (p<0.001) 2) LVEDV was decreased significantly from 103.0 ±42.8 ml/m' to 71.9±29.2 ml/m' after one month of renal transplantation. (p < 0.001) 3) LVM was decreased significantly from 265.3±107. 6 g/m to 211.2±77.3 g/m after one month of renal transplantation. (p <0.001) 4) No significant changes in EF and 9.FS were obser- ved. 5) SI and CI were decreased significantly from 65.6± 22.5 ml/m to 48.0±17.2 ml/m' and from 5.69±2.05 L/ min/m to 4.79±1.77 L/min/m after one month of renal transplantation. (p < 0.05) In conclusion, both regression of LVM and normal- ization of SI were detected as early as one month after renal transplantation. It suggests that the renal trans- plantation can improve the cardiac function and mor- phology in the early stage.
