소아에서 B 형 간염 Virus 관련 신사구체 병증

Abstract
The clinicopathological observation about 46 children with hepatitis B virus associated glomerulopathy (HBV-GP) was done, and the results were summarized as follows; 1) Membranous nephropathy (MN) was the main pathologic feature of HBV-GP in children. However, there were considerable cases in the spectrum from MN to membranoproliferative glomerulonephritis (MPGN 2) Those in whom MN was the only or predominant finding showed earlier onset age and higher incidence of accompanying nephrotic syndrome than others. 3) Those with subepithelial immune deposits of any amount revealed hepatitis B e antigenemia, while those without them didnt. 4) Some children with HBV-GP, especially those with MN, showed a tendency of spontaneous remission, mostly associated with negative conversion of hepatitis B e antigenemia. 5) Steroid therapy was relatively effective immediate- ly in more than a half of cases. However there was no evidence of its advantage during long term follow-up. In conclusion, HBV-GP in children has a wide his- topathologic spectrum from typical MN to typical MPGN, and the lesion of MN may be pathogenetically associated with hepatitis B e antigen.
