독립적 육안적 혈뇨 환아에 대한 임상 병리학적 고찰

Abstract
Isolated gross hematuria is a frequently arising clini- cal problem in children. There had been total 53 children with above problem in whom renal biopsy was perfor- med in the Department of Pediatrics, Seoul National University Childrens Hospital during the period from 1979 to 1988. A clinicopathological observation was done, and the results were as follows; 1) The mean age at the onset of gross hematuria was 8.36±3.19 yrs and the sex ratio was about 2.1:l. 2) IgA nephropathy (34 cases, 64.2 %) was the leading pathologic lesion, and minor change lesion (9 cases, 17%), thin membrane disease (3 cases) followed. 3) There were no significant differences in clinical features and laboratory findings between IgA nephropathy and other pathologic lesions, except for higher incidence of elevated serum IgA level in the former group. In conclusion, IgA nephropathy was the underlying disease in about 2/3 of children with isolated gross hematuria. There was no differential point between IgA nephropathy and others except for serum IgA level. However, most pathologic lesions including IgA ne- phropathy were those with benign clinical courses. So, we recommend careful clinical follow-up for a sufficient duration before invasive studies including renal biopsy.
