소아 낭창성 신염의 임상적 및 병리학적 고찰

Abstract
Twelve pediatric cases of lupus nephritis which were admitted to the Department of Pediatrics, Yonsei Medi- cal Center from Jan. 1, 1978 to Dec. 31, 1985 are revi- ewed. A review of the clinical and pathological findings revealed the following: 1) The male to female ratio was 1.4: 1, and in 9 cases (75%), the age of onset was 10 years or older. 2) Renal biopsies were performed in 9 cases with the following results. There were 3 cases of focal and seg- mental proliferative glomerulonephritis, 5 cases of dif- fuse proliferative glomerulonephritis, and one of mem- branous lupus nephritis. 3) Not one of the clinicopathologic findings which included age, 24 hours urine protein, WHO classifica- tion, activity index and chronicity index, could signifi- cantly predict or indicate uremia in children. 4) Of the 12 cases of lupus nephritis, there were 5 cases of mortality during the follow-up period, and 4 of these occurred within 4 months of the onset of the disease. In conclusion, when lupus nephritis is suspected, renal biopsy, tests for antinuclear antibody (ANA) and anti- DNA, and a complement profile should be performed, and followed by the appropriate treatment.
